medical history. To help keep your first visit as brief as possible, we have
posted this form below. Simply print it, fill it out at your convenience, and
bring it with you to your appointment.

your name, address and insurance carrier. It also gives us a detailed
summary of your medical and dental history, so we may determine what
medicines, procedures and treatments will work best for you.
